Weight-Loss Medication in Japan: What’s Approved, What Isn’t, and What It Costs

If you moved to Japan from the US, the UK, Australia, or almost anywhere in Europe, you have probably watched friends back home start Ozempic, Wegovy, or Mounjaro. Then you tried to find the equivalent here, and ran into a wall of Japanese-language clinic websites, contradictory information, and prices that seem to vary by a factor of five.

Here is the honest picture. Japan’s rules are genuinely different from your home country’s — not just administratively, but in how obesity itself is defined and who is allowed to prescribe what. Most of the confusion English-speaking residents run into comes from assuming the system works the way it does back home. It does not.

The short version

  • Japan defines obesity at a lower BMI threshold than Western countries — but a high BMI alone is not enough to qualify for treatment.
  • Two medicines are approved in Japan specifically for obesity: Wegovy and Zepbound. We prescribe both, on a fully self-pay basis.
  • Three others — Rybelsus, Metformin, and Mounjaro — are approved in Japan for type 2 diabetes only. Using them for weight loss is off-label and always self-pay.
  • Off-label means: thinner evidence, no public compensation if something goes wrong, and full cost out of pocket.
  • Anyone who offers you a prescription without blood work and an in-person examination is cutting corners you should not accept.

Japan draws the line at BMI 25, not 30

In most Western countries, “obesity” begins at a BMI of 30. In Japan, the Japan Society for the Study of Obesity sets the threshold at BMI 25.

That sounds like it should make treatment easier to access. It does not — because Japan draws a sharp distinction between two things that English collapses into one word.

肥満 (himan) — “obesity” as a body-size category. BMI 25 or above. This is a description, not a diagnosis. It does not by itself qualify you for anything.

肥満症 (himan-shō) — “obesity disease,” a medical diagnosis. BMI 25 or above plus at least one of eleven recognised obesity-related health disorders, or a BMI of 35 or above.

Those eleven disorders include type 2 diabetes, dyslipidaemia, hypertension, hyperuricaemia and gout, coronary artery disease, cerebral infarction, fatty liver disease, menstrual disorders, sleep apnoea, osteoarthritis affecting weight-bearing joints, and obesity-related kidney disease.

This distinction is the single most important thing to understand. A healthy 32-year-old with a BMI of 28 and no metabolic problems is himan but not himan-shō. In Japanese medicine, that person does not have a disease to treat — which is why so many clinics decline to prescribe, and why the ones that do are working outside the approved indication.

The five medicines and their status in Japan

Mounjaro and Zepbound are the same molecule. Both are tirzepatide, made by the same company. The difference is the approved indication and the packaging: Mounjaro is licensed for diabetes, Zepbound for obesity disease. Same drug, different regulatory box.

Ozempic is not on this list. Semaglutide injection for diabetes is available in Japan under that name, but the obesity-dose version is Wegovy. If a clinic offers you “Ozempic for weight loss,” that is off-label use of a diabetes medicine.

Zepbound gained a sleep apnoea indication in May 2026 — the first time a weight-management medicine has been approved in Japan for a condition other than obesity itself.

How Wegovy and Zepbound actually work in Japan

Both medicines fall under the Ministry of Health’s Optimal Use Promotion Guidelines (最適使用推進ガイドライン). You will read elsewhere that this makes them nearly impossible to obtain in Japan. That is only half the picture, and the missing half is what brings most people to this page.

Those guidelines govern what health insurance will pay for. They are not a licensing restriction on the medicine itself. A physician may prescribe Wegovy or Zepbound and charge the patient privately — and a number of clinics in Japan, including ours, do.

Private Clinic Ebisu is a fully self-pay clinic. We do not provide insurance-covered care of any kind. Every treatment here, for every patient, is paid for privately.

We prescribe only when our physician determines, after examining you in person, that treatment is appropriate. We do not prescribe on request, and we do not issue a first prescription without recent blood work. Sometimes the answer is no. Please discuss the details at your consultation.

What “off-label, self-pay” actually means

Three things follow, and you should be clear on all of them before you contact any clinic — ours included.

1. You pay the full cost. We are a 100% self-pay clinic; holding a Japanese health insurance card makes no difference to what you pay.

2. The evidence base is thinner than the marketing suggests. These medicines were studied in people with type 2 diabetes. The Japan Diabetes Society has stated that safety and effectiveness in Japanese people without type 2 diabetes have not been established.

3. A physician decides, not you. Sometimes the answer is no. If you are looking for a clinic that will issue a prescription on request, we are not it.

Drug injury relief system

Japan operates a public compensation scheme for people seriously harmed by adverse drug reactions — the Relief System for Sufferers from Adverse Drug Reactions (医薬品副作用被害救済制度).

In the event of a serious adverse reaction, you will in principle not be eligible for this system. Please proceed on that understanding.

There is also a supply ethics question. The Ministry of Health has noted that demand driven by cosmetic use has contributed to supply constraints affecting people who need these medicines for diabetes. A responsible clinic factors this in.

Costs scale with dose. Because these medicines are titrated upward over months, your cheapest month is your first month. Budget for the maintenance dose, not the starting dose — this is the single most common financial surprise patients report.

Blood work is required before we prescribe, but we do not draw blood here — please have the tests done at a nearby internal medicine clinic and bring the results to your consultation. That clinic bills you separately. We stock Wegovy up to 1.0 mg and tirzepatide up to 5 mg; if you would benefit from a higher dose than that, we will say so and help you find a clinic that can continue your treatment.

One thing you should refuse

You will find websites offering GLP-1 medicines by mail, with no blood work, no in-person examination, and sometimes no consultation at all. Some offer personal-import product sourced from overseas.

Do not do this. The Ministry of Health has warned that personally imported medicines carry no guarantee of quality, effectiveness, or safety, and carry a significant risk of counterfeits. You also have no compensation route if you are harmed.

There is a simple test for whether a clinic is treating you properly. Before your first prescription, a physician should:

  • examine you in person
  • take a full medical and medication history, including any history of eating disorders
  • ask about pregnancy plans
  • require and review recent blood work — liver function, kidney function, pancreatic enzymes, thyroid function and HbA1c are the values that matter here
  • explain the titration schedule and what side effects to expect
  • tell you when to come back
  • be willing to say no

If those things do not happen, walk away. That is not a high bar — it is the minimum.

Where to go from here

BMI 27+ with two or more obesity-related conditions, or BMI 35+
You fall inside the group Wegovy and Zepbound were approved for in Japan. If our physician agrees after examining you, this is the most straightforward case for treatment here. Everything is self-pay.

BMI 25+ with one related health condition
You have a himan-shō diagnosis but do not meet the criteria for Wegovy or Zepbound. Medication is one option among several, and not always the first. Worth a conversation.

BMI 25–27 with no health conditions
You are in the largest and most frustrated group. Whether we can help you at all is a decision our physician makes after examining you — and sometimes the answer is no. Where treatment falls outside a medicine’s approved indication, the evidence base is thinner, the cost is entirely yours, and the drug injury relief system will in principle not apply. We will still talk to you, and we will be straight with you about whether we think it is a good idea.

BMI under 25
We will almost certainly say no, and we would rather tell you that in a LINE message than after you have paid for a consultation.

在日外国人の方向けガイド

日本の減量薬 — 何が承認され、何が承認されていないのか

米国・英国・オーストラリア、あるいはヨーロッパから日本に移り住んだ方であれば、母国の友人がオゼンピックやウゴービ、マンジャロを始めるのを目にしてきたことでしょう。ところが日本で同じものを探そうとすると、日本語のクリニックサイトの壁、矛盾する情報、5倍もの開きがある価格に突き当たります。

実情を率直にお伝えします。日本のルールは母国のそれとは本質的に異なります。手続きの違いというだけでなく、肥満の定義そのもの、そして誰が何を処方できるのかという点が違います。英語話者の方が混乱される原因のほとんどは、母国と同じ仕組みだと想定してしまうことにあります。日本は違います。

要点

  • 日本は欧米より低いBMI基準で肥満を定義しています。ただしBMIが高いだけでは治療の対象になりません。
  • 肥満症治療薬として承認されているのは2剤(ウゴービ・ゼップバウンド)。当院では全額自費でご提供しています。
  • 他の3剤(リベルサス・メトホルミン・マンジャロ)は2型糖尿病に対してのみ承認されています。減量目的の使用は適応外であり、常に自費です。
  • 適応外とは、エビデンスが薄く、健康被害が生じても公的救済の対象外で、費用は全額自己負担ということです。
  • 血液検査も対面診察もなく処方を提案してくる医療機関は、受け入れるべきでない手抜きをしています。

日本の基準はBMI 30ではなくBMI 25

欧米の多くの国では「肥満」はBMI 30以上を指します。日本肥満学会が定める基準はBMI 25です。

基準が低いなら治療を受けやすいはずだ、と思われるかもしれません。実際は逆です。日本では、英語では1語にまとめられてしまう2つの概念が明確に区別されているためです。

肥満 — 体格の分類。BMI 25以上。これは状態の記述であって診断名ではなく、それ自体では治療の対象になりません。

肥満症 — 医学的な診断名。BMI 25以上に加えて11種類の肥満関連健康障害のうち1つ以上を有する場合、またはBMI 35以上の高度肥満。

11種類の健康障害には、2型糖尿病、脂質異常症、高血圧、高尿酸血症・痛風、冠動脈疾患、脳梗塞、脂肪肝、月経異常、睡眠時無呼吸症候群、変形性関節症(荷重関節)、肥満関連腎臓病が含まれます。

この区別が最も重要な点です。BMI 28で代謝異常のない健康な32歳の方は「肥満」ではあっても「肥満症」ではありません。日本の医療では、その方は治療すべき疾患を有していないと判断されます。多くのクリニックが処方を断る理由、そして処方するクリニックが承認された適応の外で診療している理由がここにあります。

5剤の日本での位置づけ

マンジャロとゼップバウンドは同一成分です。いずれもチルゼパチドで、製造元も同じです。違うのは承認された適応と製品としての区分だけで、マンジャロは糖尿病、ゼップバウンドは肥満症に対して承認されています。同じ薬が、別の規制区分に入っているということです。

オゼンピックはこの表にありません。糖尿病用のセマグルチド注射剤は日本でもその名称で使用できますが、肥満症用量の製剤はウゴービです。「減量目的でオゼンピックを」と提案された場合、それは糖尿病治療薬の適応外使用です。

ゼップバウンドは2026年5月に睡眠時無呼吸症候群の適応を取得しました。体重管理薬が肥満症以外の疾患で日本の承認を得た初めての例です。

ウゴービ・ゼップバウンドの日本での実情

この2剤は厚生労働省の最適使用推進ガイドラインの対象です。「日本ではほぼ入手できない」という情報をご覧になったかもしれません。それは事実の半分で、残りの半分こそ、多くの方がこのページに辿り着く理由です。

このガイドラインが定めているのは、健康保険が何を支払うかという範囲です。医薬品そのものの使用を制限する規定ではありません。医師が処方し、患者様に自費でご負担いただくことは可能で、当院を含め日本国内の複数の医療機関がそのように運用しています。

「適応外・自費」が実際に意味すること

3つの帰結があります。当院を含め、どの医療機関に連絡される前にも把握しておいてください。

1. 費用は全額ご負担いただきます。当院は100%自費診療のクリニックです。日本の健康保険証をお持ちの場合も費用は変わりません。

2. エビデンスは宣伝ほど厚くありません。これらの薬剤は2型糖尿病の患者を対象に研究されたものです。日本糖尿病学会は、2型糖尿病を有さない日本人における安全性・有効性は確認されていないと明記しています。

3. 判断するのは医師です。お断りする場合もあります。ご希望どおりに処方する医療機関をお探しであれば、当院は該当しません。

医薬品副作用被害救済制度について

日本には、医薬品の副作用による重篤な健康被害に対する公的救済制度(医薬品副作用被害救済制度)があります。

万が一重篤な副作用が出た場合には、原則として国の医薬品副作用被害救済制度の対象外となります。この点をご理解のうえ、ご検討ください。

供給倫理の問題もあります。厚生労働省は、美容目的の需要増加が、糖尿病治療として必要とする患者への供給に影響を与えていると指摘しています。責任ある医療機関はこの点も考慮します。

費用は用量に応じて上がります。数か月かけて段階的に増量していくため、最も安いのは開始月です。開始時の用量ではなく維持用量を前提にご予算をお考えください。患者様から最も多くいただくご指摘がこの点です。

当院では診察が無料のため、上記が当院でのお支払い総額(税込)となります。処方前に血液検査が必須ですが、当院では採血を行っておりません。お近くの内科で検査を受けていただき、結果を診察時にお持ちください。検査費用は当該医療機関へのお支払いとなります。取り扱いはウゴービ1.0mgまで、チルゼパチド5mgまでです。それ以上の用量が適していると判断される場合はその旨をお伝えし、治療を継続できる医療機関をご案内します。

料金の詳細と開示事項:料金とリスクについて

断るべきこと

血液検査も対面診察もなく、場合によっては問診すらなく、GLP-1製剤を郵送するウェブサイトが存在します。海外から個人輸入した製品を扱うところもあります。

利用しないでください。厚生労働省は、個人輸入された医薬品は品質・有効性・安全性が保証されず、偽造品のリスクが大きいと注意喚起しています。健康被害が生じても救済を受ける手段がありません。

適切な診療を行っている医療機関かどうかは、簡単に見分けられます。最初の処方の前に、医師は以下を行うはずです。

  • 対面で診察する
  • 摂食障害の既往を含め、既往歴と服薬歴を確認する
  • 妊娠の予定を確認する
  • 直近の血液検査の結果を必須とし、内容を説明する(肝機能・腎機能・膵酵素・甲状腺機能・HbA1cが関連項目です)
  • 増量スケジュールと想定される副作用を説明する
  • 次回いつ受診すべきかを伝える
  • 必要であれば「処方しない」と言える

これらが行われないのであれば、その医療機関は避けてください。高い水準の話ではなく、最低限の話です。

ご自身の場合はどうすべきか

BMI 27以上かつ肥満関連疾患を2つ以上、またはBMI 35以上の方
ウゴービ・ゼップバウンドが日本で承認された対象に該当します。診察のうえ医師が適切と判断した場合、当院での治療が最も進めやすいケースです。費用は全額自費となります。

BMI 25以上かつ関連疾患が1つの方
肥満症の診断には該当しますが、ウゴービ・ゼップバウンドの基準は満たしません。薬物療法は選択肢の一つであり、常に最初の選択とは限りません。一度ご相談ください。

BMI 25〜27で関連疾患のない方
最も人数が多く、最ももどかしい思いをされている層です。当院でお力になれるかどうかは、医師が診察したうえで判断します。お断りする場合もあります。承認された適応の外にあたる治療では、エビデンスは薄く、費用は全額ご負担いただき、救済制度も原則適用されません。それでもご相談はお受けしますし、当院として妥当と考えるかどうかを率直にお伝えします。

BMI 25未満の方
ほぼ確実にお断りすることになります。診察料をお支払いいただいた後ではなく、LINEのメッセージの段階でお伝えしたいと考えています。
