Mounjaro in Japan: Cost, Access, and Why It Is Off-Label
Mounjaro is the most-searched weight-loss injection among English speakers in Japan. It is also the one most often described inaccurately. Here is what it actually is under Japanese rules.
The short version
- Mounjaro is tirzepatide — the same molecule as Zepbound.
- In Japan it is approved for type 2 diabetes only. Prescribing it for weight loss is off-label.
- It is not approved for weight loss in the United States either. There, the weight-management brand is Zepbound.
- Off-label means: full cost, thinner evidence, and no public compensation if something goes seriously wrong.
- If you meet Japan’s obesity-disease criteria, Zepbound may be the more appropriate choice. We will tell you if so.
What Mounjaro is
Mounjaro is a once-weekly injection containing tirzepatide, made by Eli Lilly. Unlike semaglutide, which acts on the GLP-1 receptor alone, tirzepatide acts on two gut-hormone receptors — GIP and GLP-1. It increases insulin release when blood sugar is high, slows stomach emptying, and reduces appetite.
In Japan, its approved indication is type 2 diabetes. Nothing else.
Mounjaro and Zepbound are the same drug
This confuses almost everyone, so it is worth being precise.
Eli Lilly markets tirzepatide under two names, with two separate regulatory approvals:

Same molecule, same pen, same dose strengths. The difference is which indication each brand was approved under.
A correction worth making. You may see Japanese clinic websites claiming that “Mounjaro is approved for weight loss by the FDA and in Europe.” That is not accurate for the United States. In the US, Mounjaro holds a diabetes approval only; the weight-management approval belongs to Zepbound. If a clinic tells you otherwise, that is a reason to be careful about what else they are telling you.
What “off-label” means for you
Prescribing an approved medicine outside its approved indication is legal in Japan — it is a decision within a physician’s professional discretion. But it has real consequences, and you should understand all four before agreeing to it.
1. You pay the entire cost. We are a 100% self-pay clinic and this treatment is not covered by insurance under any circumstances.
2. The evidence is thinner than the marketing suggests. Mounjaro’s trials studied people with type 2 diabetes. The Japan Diabetes Society has stated that safety and effectiveness in Japanese people without type 2 diabetes have not been established.
3. Public compensation does not apply. See the disclosures below.
4. There is an approved alternative. If you meet Japan’s obesity-disease criteria, Zepbound is the medicine actually approved for that purpose — and it contains the same active ingredient. We stock both. Our physician will tell you which is more appropriate for your situation.
Blood tests are required before we prescribe
Impaired kidney function, raised pancreatic enzymes, and thyroid abnormalities all rule out treatment, and none of them can be found by examination alone.
We do not draw blood at this clinic. Please have the tests done at a nearby internal medicine clinic (内科) and bring the printed results. Results from the past 12 months are acceptable: liver function (AST, ALT, γ-GTP), kidney function (creatinine, eGFR), pancreatic enzymes (amylase, lipase), thyroid function (TSH, FT4), and HbA1c with fasting glucose.

2.5 mg is a starting dose used for the first four weeks; it is not intended as a maintenance dose. Consultations are free, so these are your total costs here, tax included. Full price list.
We do not stock tirzepatide above 5 mg. Overseas maintenance doses go considerably higher. If you reach our ceiling and would benefit from more, we will say so and help you find somewhere that can continue your treatment.
Side effects
Nausea, constipation, diarrhoea, and reduced appetite are common — worst in the first weeks and after each increase, and usually settling. Smaller portions eaten slowly help more than anything else.
Stop and seek medical attention immediately for severe, persistent abdominal pain radiating to the back (possible pancreatitis), or upper-right abdominal pain with fever (possible gallbladder problems).
Weight regain after stopping is expected, not a failure of treatment. Discuss a long-term plan with us before you start.
Required disclosures
This is an off-label, self-pay treatment
Mounjaro is not approved in Japan for weight loss. It is approved in Japan as a treatment for type 2 diabetes. Prescribing it for weight management is an off-label use. Private Clinic Ebisu is a 100% self-pay clinic and does not provide insurance-covered care of any kind.

We purchase Mounjaro through licensed Japanese pharmaceutical wholesalers, as genuine product manufactured and distributed in Japan by Eli Lilly Japan K.K. We do not use personal imports or overseas-sourced product, and we will not prescribe or administer medicine you have imported yourself.
Are there approved alternatives in Japan?
Yes. Japan has two medicines approved specifically for obesity disease (肥満症): Wegovy (semaglutide) and Zepbound (tirzepatide). Zepbound contains the same active ingredient as Mounjaro. We stock both. Whether one of these is more appropriate for you is something to discuss with our physician at your consultation.
Safety information from other countries
The active ingredient in Mounjaro is tirzepatide. In the United States, tirzepatide is marketed under two separate brand names with two separate approvals: Mounjaro is approved for type 2 diabetes only, while Zepbound is the brand approved for chronic weight management and for moderate-to-severe obstructive sleep apnoea. Mounjaro itself does not hold a weight-management approval in the United States.
US prescribing information for tirzepatide carries a boxed warning regarding the risk of thyroid C-cell tumours, based on rodent studies. Reported adverse reactions include nausea, vomiting, diarrhoea, constipation, abdominal pain, reduced appetite, acute pancreatitis, gallbladder disease, acute kidney injury following severe dehydration, and hypoglycaemia when combined with insulin or sulfonylureas. Do not use this medicine if you
are pregnant, breastfeeding or planning pregnancy; have a personal or family history of medullary thyroid carcinoma; have Multiple Endocrine Neoplasia syndrome type 2 (MEN2); have a history of pancreatitis; have severe gastrointestinal disease or gastroparesis; or have a history of an eating disorder.
Drug injury relief system
In the event of a serious adverse reaction, you will in principle not be eligible for Japan’s Relief System for Sufferers from Adverse Drug Reactions (医薬品副作用被害救済制度) or the Relief System for Infections Acquired through Biological Products (生物由来製品感染等被害救済制度). Where Mounjaro is used for weight management, these public compensation schemes are not available to you. Please make sure you understand this before proceeding.

マンジャロ — 日本での費用・入手方法と、適応外である理由
マンジャロは、在日外国人が最も多く検索する減量目的の注射薬です。同時に、最も不正確に説明されている薬でもあります。日本の制度上、実際にどのような位置づけなのかを整理します。
要点
- マンジャロの成分はチルゼパチド。ゼップバウンドと同一成分です。
- 日本では2型糖尿病に対してのみ承認されています。減量目的の処方は適応外使用です。
- 米国でも減量目的では承認されていません。米国で体重管理の承認を持つのはゼップバウンドです。
- 適応外とは、費用は全額自己負担、エビデンスは薄く、重篤な健康被害が生じても公的救済の対象外ということです。
- 日本の肥満症の基準を満たす方には、ゼップバウンドの方が適している場合があります。該当する場合はその旨をお伝えします。
マンジャロとは
マンジャロはチルゼパチドを含む週1回の皮下注射剤で、製造元はイーライリリーです。GLP-1受容体のみに作用するセマグルチドと異なり、チルゼパチドはGIPとGLP-1という2つの消化管ホルモン受容体に作用します。血糖が高いときにインスリン分泌を促し、胃排出を遅らせ、食欲を抑えます。
日本での承認適応は2型糖尿病のみです。
マンジャロとゼップバウンドは同じ薬です
ほぼ全員が混乱する点なので、正確に整理します。
イーライリリーはチルゼパチドを2つの名称で、2つの別々の承認のもとに販売しています。

成分も、ペンの device も、規格も同じです。違いは、どの適応で承認を取得したかだけです。
訂正しておきたい点があります。「マンジャロは米国FDAや欧州で減量目的に承認されている」と記載しているクリニックのサイトを見かけることがあります。米国については正確ではありません。米国においてマンジャロが持つのは糖尿病の承認のみで、体重管理の承認はゼップバウンドのものです。そのように説明する医療機関があれば、他の説明についても慎重に確認された方がよいと思います。
「適応外」が意味すること
承認された医薬品を承認適応の外で処方することは、日本において違法ではありません。医師の裁量に属する判断です。ただし現実的な帰結があり、同意される前に4点すべてをご理解ください。
1. 費用は全額ご負担いただきます。当院は100%自費診療のクリニックであり、この治療がいかなる場合も保険適用となることはありません。
2. エビデンスは宣伝ほど厚くありません。マンジャロの試験は2型糖尿病の患者を対象としたものです。日本糖尿病学会は、2型糖尿病を有さない日本人における安全性・有効性は確認されていないと明記しています。
3. 公的救済制度は適用されません。下記の開示事項をご確認ください。
4. 承認された代替薬があります。日本の肥満症の基準を満たす方には、その目的で実際に承認されているゼップバウンドがあり、有効成分はマンジャロと同一です。当院は両方を取り扱っています。どちらが適しているかは診察時に医師がお伝えします。
処方前に血液検査が必須です
腎機能の低下、膵酵素の上昇、甲状腺の異常はいずれも処方できない理由となりますが、診察だけでは分かりません。
当院では採血を行っておりません。お近くの内科で検査を受けていただき、結果の用紙をお持ちください。過去12か月以内の結果で結構です。項目は、肝機能（AST・ALT・γ-GTP）、腎機能（クレアチニン・eGFR）、膵酵素（アミラーゼ・リパーゼ）、甲状腺機能（TSH・FT4）、HbA1cと空腹時血糖です。

当院ではチルゼパチド5mgまでの取り扱いです。海外の維持用量はこれよりかなり高くなります。当院の上限に達し、さらに増量した方がよいと判断される場合はその旨をお伝えし、治療を継続できる医療機関をご案内します。
副作用
悪心、便秘、下痢、食欲低下はよくみられます。開始直後と増量直後に強く、通常は落ち着きます。一度に食べる量を減らし、ゆっくり食べることが最も効果的です。
直ちに使用を中止し受診してください：背部に放散する強く持続する腹痛（膵炎の可能性）、右上腹部痛と発熱（胆嚢の異常の可能性）。
中止後の体重再増加は想定される経過であり、治療の失敗ではありません。開始前に長期的な方針をご相談ください。
必須の開示事項
適応外・自費診療です
マンジャロは、日本国内において減量目的での使用については承認されていません。日本では2型糖尿病の治療薬として承認されています。減量目的での処方は適応外使用にあたります。プライベートクリニック恵比寿は100%自費診療のクリニックであり、保険診療は一切行っておりません。
入手経路
マンジャロは国内の医薬品卸業者を通じて、日本イーライリリー株式会社が国内で製造販売する正規品を仕入れています。個人輸入品・海外調達品は一切使用しておりません。患者様ご自身が輸入された医薬品の処方・投与もお受けできません。
国内の承認医薬品等の有無
あります。日本には肥満症に対して承認された医薬品が2剤あります。ウゴービ（セマグルチド）とゼップバウンド（チルゼパチド）です。ゼップバウンドの有効成分はマンジャロと同一です。当院は両方を取り扱っています。どちらが適しているかは診察時に医師とご相談ください。
諸外国における安全性等に係る情報
マンジャロの有効成分はチルゼパチドです。米国では、チルゼパチドは2つの製品名で2つの別々の承認のもとに販売されています。マンジャロは2型糖尿病に対してのみ承認されており、体重管理の承認を持つのはゼップバウンド（および中等症以上の閉塞性睡眠時無呼吸症候群）です。米国においてマンジャロ自体は体重管理の承認を有していません。
米国の添付情報では、げっ歯類試験に基づく甲状腺C細胞腫瘍のリスクに関する枠組み警告が付されています。報告されている副作用には、悪心、嘔吐、下痢、便秘、腹痛、食欲減退、急性膵炎、胆嚢疾患、重度脱水に伴う急性腎障害、インスリンやスルホニル尿素薬併用時の低血糖があります。詳細は各国規制当局（米国FDA、EMA）の公式サイトをご確認ください。
以下に該当する方は使用できません
妊娠中・授乳中の方、妊娠を計画中の方／甲状腺髄様癌の既往歴またはご家族に既往のある方／多発性内分泌腫瘍症2型（MEN2）の方／膵炎の既往歴のある方／重度の消化器疾患または胃不全麻痺のある方／摂食障害の既往歴のある方。
医薬品副作用被害救済制度について
万が一重篤な副作用が出た場合には、原則として国の医薬品副作用被害救済制度および生物由来製品感染等被害救済制度の対象外となります。マンジャロを減量目的で使用する場合、これらの公的救済制度を利用することはできません。十分にご理解のうえ、ご検討ください。
