Wegovy and Zepbound in Japan: How to Actually Get Them
Search for these two medicines in English and you will find the same sentence repeated everywhere: almost impossible to get in Japan. That is half true, and the half that gets left out is the part that matters.
The short version
- Both are properly approved in Japan — for obesity disease (肥満症), not for general weight loss.
- Getting them on insurance is genuinely difficult: specialist hospitals only, strict criteria, and months of supervised diet therapy first.
- Getting them self-pay is not. The restrictions govern reimbursement, not the medicine itself.
- We prescribe both, fully self-pay, after an examination and recent blood work.
- We stock Wegovy up to 1.0 mg and Zepbound up to 5 mg. If you need more than that, we will tell you.
What these two medicines are
Wegovy is semaglutide — a GLP-1 receptor agonist, given as a once-weekly injection. It mimics a gut hormone that signals fullness to the brain and slows how quickly the stomach empties.
Zepbound is tirzepatide, which acts on two receptors: GLP-1 and GIP. In head-to-head trials this dual action has produced greater average weight loss than semaglutide.
Both are the obesity-indication versions of medicines you may know by other names. Semaglutide for diabetes is Ozempic; tirzepatide for diabetes is Mounjaro. Same molecules, different approvals, different boxes.
What “approved for obesity disease” means in Japan
Japan approved these medicines for a narrower group than you might expect. The approved indication requires hypertension, dyslipidaemia, or type 2 diabetes, insufficient results from diet and exercise, and either:
- BMI 27 or above with two or more obesity-related health disorders, or
- BMI 35 or above
The recognised obesity-related disorders are type 2 diabetes, dyslipidaemia, hypertension, hyperuricaemia and gout, coronary artery disease, cerebral infarction, fatty liver disease, menstrual disorders, sleep apnoea, osteoarthritis of weight-bearing joints, and obesity-related kidney disease.
In May 2026, Zepbound gained an additional indication for moderate-to-severe obstructive sleep apnoea — the first time a weight-management medicine has been approved in Japan for something other than obesity itself.
Why insurance access is so restricted
Both fall under the Ministry of Health’s Optimal Use Promotion Guidelines. Under insurance, a clinic must be registered with the manufacturer’s proper-use committee — which generally requires a full-time board-certified specialist (diabetes, endocrinology, or cardiology), a registered dietitian on staff, and accredited teaching-facility status.
In practice: university hospitals and large regional hospitals. And even there, patients must complete a documented period of dietitian-supervised diet and exercise therapy — several months at minimum — and be found insufficiently improved, before medication can start. Coverage then runs to a maximum of 72 weeks.
The part nobody explains in English
Those guidelines govern what health insurance will pay for. They are not a licensing restriction on the medicine itself.
A physician may prescribe Wegovy or Zepbound and charge the patient privately. A number of clinics in Japan do exactly that, and we are one of them.
Private Clinic Ebisu is a 100% self-pay clinic. We do not provide insurance-covered care of any kind. Holding a Japanese health insurance card makes no difference to what you pay.
We prescribe only after an in-person examination, and only with recent blood work in hand. We do not prescribe on request. Sometimes the answer is no, and we would rather tell you that early.
Blood tests are required before we prescribe
Several of the things that rule a person out — impaired kidney function, raised pancreatic enzymes, thyroid abnormalities — cannot be found by examination alone.
We do not draw blood at this clinic. Please have the tests done at a nearby internal medicine clinic (内科) and bring the printed results with you. Results from the past 12 months are acceptable. What we need:
- Liver function — AST, ALT, γ-GTP
- Kidney function — creatinine, eGFR
- Pancreatic enzymes — amylase, lipase
- Thyroid function — TSH, FT4
- HbA1c and fasting blood glucose

Doses, and where we stop
Both medicines are titrated upward in steps, usually every four weeks, to let side effects settle between increases.

This is worth knowing before you start. The maintenance doses used in the overseas trials are higher than the doses we hold. If you reach our ceiling and would benefit from going further, we will say so and help you find a clinic that can continue your treatment. Raise it at your first consultation so you can plan around it.
What to expect, and what to watch for
Nausea, constipation, diarrhoea, and reduced appetite are common, most noticeable in the first weeks and after each dose increase, and usually settle. Eating smaller portions more slowly helps more than anything else.
Stop and seek medical attention immediately for severe, persistent abdominal pain radiating to the back (possible pancreatitis), or for pain in the upper right abdomen with fever (possible gallbladder problems).
Weight regain after stopping is expected, not a failure. These medicines treat obesity as a chronic condition; the effect lasts while treatment does. Please talk to us about a long-term plan before you start, not after.

Wegovy: purchased through licensed Japanese pharmaceutical wholesalers, as genuine product manufactured and distributed by Novo Nordisk Pharma Ltd.
Zepbound: purchased through licensed Japanese pharmaceutical wholesalers, as genuine product manufactured and distributed by Eli Lilly Japan K.K.
We do not use personal imports or overseas-sourced product, and we will not prescribe or administer medicine you have imported yourself.
Safety information from other countries
Semaglutide and tirzepatide are both approved for chronic weight management in the United States and other countries. US prescribing information for both carries a boxed warning regarding the risk of thyroid C-cell tumours, based on rodent studies. Reported adverse reactions include nausea, vomiting, diarrhoea, constipation, abdominal pain, reduced appetite, acute pancreatitis, gallbladder disease, acute kidney injury following severe dehydration, and hypoglycaemia when combined with insulin or sulfonylureas. Do not use these medicines if you
are pregnant, breastfeeding or planning pregnancy; have a personal or family history of medullary thyroid carcinoma; have Multiple Endocrine Neoplasia syndrome type 2 (MEN2); have a history of pancreatitis; have severe gastrointestinal disease or gastroparesis; or have a history of an eating disorder.

In the event of a serious adverse reaction, you will in principle not be eligible for Japan’s Relief System for Sufferers from Adverse Drug Reactions (医薬品副作用被害救済制度) or the Relief System for Infections Acquired through Biological Products (生物由来製品感染等被害救済制度). Please proceed on this understanding.

ウゴービとゼップバウンド — 日本での実際の入手方法
この2剤を英語で検索すると、どこにも同じ一文が並んでいます。「日本ではほぼ入手できない」。半分は事実です。そして省かれているもう半分こそが重要な部分です。
要点
- どちらも日本で正式に承認されています。ただし対象は肥満症であり、一般的な減量ではありません。
- 保険で受けるのは確かに困難です。専門病院に限られ、要件も厳しく、事前に数か月の指導下の食事療法が必要です。
- 自費で受けるのは困難ではありません。制限は保険償還にかかるもので、薬剤の使用そのものを縛る規定ではないためです。
- 当院では、診察と直近の血液検査結果を確認したうえで、全額自費にて両剤を処方しています。
- 取り扱いはウゴービ1.0mgまで、ゼップバウンド5mgまでです。それ以上が必要な場合はその旨をお伝えします。
2剤の違い
ウゴービはセマグルチド。GLP-1受容体作動薬で、週1回の皮下注射です。満腹感を脳に伝える消化管ホルモンを模倣し、胃の内容物が排出される速度を緩やかにします。
ゼップバウンドはチルゼパチド。GLP-1に加えてGIPという2つの受容体に作用します。直接比較した試験では、この二重作用によりセマグルチドを上回る平均減量幅が示されています。
いずれも、別の名称でご存じかもしれない薬剤の「肥満症版」です。糖尿病用のセマグルチドはオゼンピック、糖尿病用のチルゼパチドはマンジャロ。同じ成分で、承認が違い、製品としての区分が違うということです。
日本での「肥満症に対する承認」の中身
日本での承認対象は、想像されるより狭い範囲です。高血圧・脂質異常症・2型糖尿病のいずれかを有し、食事・運動療法で効果不十分であり、かつ以下のいずれかに該当することが要件です。
- BMI 27以上かつ肥満関連の健康障害を2つ以上有する
- BMI 35以上
肥満関連の健康障害とは、2型糖尿病、脂質異常症、高血圧、高尿酸血症・痛風、冠動脈疾患、脳梗塞、脂肪肝、月経異常、睡眠時無呼吸症候群、変形性関節症（荷重関節）、肥満関連腎臓病を指します。
2026年5月、ゼップバウンドは中等症以上の閉塞性睡眠時無呼吸症候群の適応を追加取得しました。体重管理薬が肥満症以外の疾患で日本の承認を得た初めての例です。
保険での入手が難しい理由
2剤とも厚生労働省の最適使用推進ガイドラインの対象です。保険診療で処方するには、製造販売業者の適正使用推進委員会に登録された医療機関である必要があり、糖尿病・内分泌・循環器いずれかの専門医が常勤し、管理栄養士が在籍し、教育研修施設であることが概ね求められます。
実質的には大学病院と地域基幹病院に限られます。そのうえ患者側も、管理栄養士の指導下で食事・運動療法を最低数か月行った記録があり、効果不十分と判定されて初めて薬物療法を開始できます。保険適用は最長72週です。
英語ではどこにも書かれていない部分
このガイドラインが定めているのは、健康保険が何を支払うかという範囲です。薬剤の使用そのものを制限する規定ではありません。
医師が処方し、患者様に自費でご負担いただくことは可能です。日本国内の複数の医療機関がそのように運用しており、当院もその一つです。

処方前に血液検査が必須です
腎機能の低下、膵酵素の上昇、甲状腺の異常など、処方できない理由となる所見は診察だけでは分かりません。
当院では採血を行っておりません。お近くの内科で検査を受けていただき、結果の用紙をお持ちください。過去12か月以内の結果で結構です。必要な項目は以下のとおりです。

開始前に知っておいていただきたい点です。海外の試験で用いられた維持用量は、当院の取扱用量より高いものです。当院の上限に達し、さらに増量した方がよいと判断される場合はその旨をお伝えし、治療を継続できる医療機関をご案内します。初診時にご相談ください。
経過と、注意すべき症状
悪心、便秘、下痢、食欲低下はよくみられます。開始直後と増量直後に強く出やすく、通常は落ち着きます。一度に食べる量を減らし、ゆっくり食べることが最も効果的です。
直ちに使用を中止し受診してください：背部に放散する強く持続する腹痛（膵炎の可能性）、右上腹部痛と発熱（胆嚢の異常の可能性）。
中止後の体重再増加は想定される経過であり、失敗ではありません。これらの薬剤は肥満を慢性疾患として扱うもので、効果は治療を続けている間続きます。開始後ではなく開始前に、長期的な方針をご相談ください。
必須の開示事項
自費診療です
ウゴービ・ゼップバウンドは日本国内で肥満症の治療薬として承認されています（ゼップバウンドは中等症以上の閉塞性睡眠時無呼吸症候群にも承認）。プライベートクリニック恵比寿は100%自費診療のクリニックであり、保険診療は一切行っておりません。処方は、医師が対面で診察したうえで適切と判断した場合に限ります。

諸外国における安全性等に係る情報
セマグルチド・チルゼパチドはいずれも米国その他の国で慢性的な体重管理に対して承認されています。米国の添付情報では両剤とも、げっ歯類試験に基づく甲状腺C細胞腫瘍のリスクに関する枠組み警告が付されています。報告されている副作用には、悪心、嘔吐、下痢、便秘、腹痛、食欲減退、急性膵炎、胆嚢疾患、重度脱水に伴う急性腎障害、インスリンやスルホニル尿素薬併用時の低血糖があります。詳細は各国規制当局（米国FDA、EMA）の公式サイトをご確認ください。
以下に該当する方は使用できません
妊娠中・授乳中の方、妊娠を計画中の方／甲状腺髄様癌の既往歴またはご家族に既往のある方／多発性内分泌腫瘍症2型（MEN2）の方／膵炎の既往歴のある方／重度の消化器疾患または胃不全麻痺のある方／摂食障害の既往歴のある方。
