Metformin for Weight Loss in Japan: The Honest Version
Metformin is the oldest and cheapest medicine on our list — ¥4,800 for thirty tablets, against ¥44,800 a month for the strongest injection. That gap invites a reasonable question: is it worth trying first? Sometimes. But you should know what you are and are not getting.
The short version
- Metformin has been used for type 2 diabetes for over sixty years. Its safety profile is very well characterised.
- In Japan it is approved for type 2 diabetes only. Weight-loss use is off-label.
- It is not approved for weight management in the US, the EU, or the UK either. Off-label everywhere.
- Weight loss is modest — typically a few kilograms, well below GLP-1 medicines.
- It carries a rare but serious risk (lactic acidosis) that makes kidney function testing essential.
What metformin does
Metformin works differently from the GLP-1 medicines. Rather than suppressing appetite through gut-hormone signalling, it reduces the amount of glucose your liver produces and improves how sensitive your tissues are to insulin.
The weight effect is a secondary consequence of that metabolic change, not the drug’s main mechanism. This is precisely why the effect is smaller.
Being straight about the size of the effect
In studies of people without diabetes, metformin has produced average weight reductions in the range of a few kilograms over months — meaningful for some people, but a different order of magnitude from what the GLP-1 injections achieve.
If you are expecting metformin to do what you have seen Wegovy or Mounjaro do, it will not. We would rather say that now than take your money and have you conclude in three months that medication does not work for you.
Where metformin can genuinely make sense: if you have insulin resistance or prediabetes, if you have PCOS, if the GLP-1 medicines are financially out of reach, or if you cannot tolerate them. Our physician will tell you which of these applies to you — including if the answer is none.
Nobody has approved this for weight loss
This is worth stating plainly, because it is different from the other medicines we prescribe.
Rybelsus and Mounjaro are at least approved for weight management somewhere — semaglutide as Wegovy, tirzepatide as Zepbound. Metformin holds a weight-management approval in no major market. In Japan, the US, the EU, and the UK, it is a diabetes medicine. Every weight-loss use of it, anywhere in the world, is off-label.
The safety point you cannot skip
Metformin’s serious risk is lactic acidosis — a build-up of lactic acid in the blood. It is rare, but it can be fatal, and it carries a boxed warning in US prescribing information.
Risk rises substantially with impaired kidney function, liver disease, heart failure, heavy alcohol use, severe dehydration, and around procedures using iodinated contrast dye (some CT scans).
This is why we require blood work. Kidney function is not something an examination can assess. Prescribing metformin without knowing your creatinine and eGFR is not acceptable practice.
Long-term use can also cause vitamin B12 deficiency, which may need periodic checking if you stay on it.
Blood tests are required before we prescribe
We do not draw blood at this clinic. Please have the tests done at a nearby internal medicine clinic (内科) and bring the printed results. Results from the past 12 months are acceptable: liver function (AST, ALT, γ-GTP), kidney function (creatinine, eGFR), pancreatic enzymes (amylase, lipase), thyroid function (TSH, FT4), and HbA1c with fasting glucose.

Side effects
Digestive symptoms are the common ones: diarrhoea, nausea, abdominal discomfort, and a metallic taste. They are usually worst at the start and often improve. Taking tablets with food helps considerably.
Seek medical attention immediately if you develop unusual muscle pain, difficulty breathing, unusual drowsiness, severe abdominal pain, or feel unusually cold — these can be signs of lactic acidosis.

This is an off-label, self-pay treatment
Metformin is approved in Japan as a treatment for type 2 diabetes. It is not approved in Japan for weight loss. Prescribing it for weight management is an off-label use. Private Clinic Ebisu is a 100% self-pay clinic and does not provide insurance-covered care of any kind.

We purchase metformin through licensed Japanese pharmaceutical wholesalers, as genuine product approved by Japan’s Ministry of Health, Labour and Welfare. We do not use personal imports or overseas-sourced product, and we will not prescribe or administer medicine you have imported yourself.
Are there approved alternatives in Japan?
Yes. Wegovy (semaglutide) and Zepbound (tirzepatide) are approved in Japan for obesity disease (肥満症). We stock both. Discuss with our physician whether one of these would be more appropriate for you.
Safety information from other countries
Metformin is not approved for weight management in the United States, the European Union, or the United Kingdom either. In every major market it is approved as a diabetes medicine. Any use for weight loss is off-label everywhere, and the evidence for weight loss is modest compared with GLP-1 receptor agonists.
US prescribing information carries a boxed warning regarding lactic acidosis — a rare but potentially fatal complication, with elevated risk in people who have impaired kidney function, liver disease, or heart failure, who consume alcohol heavily, or who undergo procedures involving iodinated contrast media. Other reported effects include diarrhoea, nausea, abdominal discomfort, metallic taste, and vitamin B12 deficiency with long-term use, which may require periodic monitoring. Do not use this medicine if you
have significantly impaired kidney function; have severe liver disease; have heart failure requiring treatment; consume alcohol heavily; are pregnant, breastfeeding or planning pregnancy; or are scheduled for a procedure involving iodinated contrast media. In the event of a serious adverse reaction, you will in principle not be eligible for Japan’s Relief System for Sufferers from Adverse Drug Reactions (医薬品副作用被害救済制度) or the Relief System for Infections Acquired through Biological Products (生物由来製品感染等被害救済制度).

メトホルミンによる減量 — 誇張のない説明
メトホルミンは、当院で扱う中で最も古く、最も安価な薬です。30錠で4,800円。最も強力な注射剤の月額44,800円と比べると、「まずこれから試すべきでは」という疑問が自然に湧きます。場合によってはそのとおりです。ただし、何が得られて何が得られないかを知っておいてください。
要点
- 60年以上にわたり2型糖尿病に使われてきた薬で、安全性の特性はよく分かっています。
- 日本では2型糖尿病に対してのみ承認されています。減量目的の使用は適応外です。
- 米国・EU・英国でも体重管理の承認はありません。世界中どこでも適応外です。
- 減量効果は穏やかです。数kg程度が典型で、GLP-1製剤には大きく及びません。
- まれですが重篤なリスク（乳酸アシドーシス）があり、腎機能の検査が不可欠です。
メトホルミンの働き
メトホルミンはGLP-1製剤とは異なる仕組みで働きます。消化管ホルモンのシグナルで食欲を抑えるのではなく、肝臓が産生するブドウ糖の量を減らし、組織のインスリン感受性を改善します。
体重への影響は、その代謝的な変化に伴う二次的な結果であって、この薬の主たる作用機序ではありません。効果が小さいのはまさにこのためです。
効果の大きさについて率直に
糖尿病のない方を対象とした研究では、数か月で平均数kg程度の減量が報告されています。人によっては意味のある数字ですが、GLP-1注射剤が達成する水準とは桁が違います。
ウゴービやマンジャロで見たような結果をメトホルミンに期待されているのであれば、それは実現しません。お代をいただいた3か月後に「自分には薬が効かない」と結論づけられるより、今お伝えする方がよいと考えています。
メトホルミンが本当に意味を持つのは、インスリン抵抗性や前糖尿病がある場合、多嚢胞性卵巣症候群（PCOS）がある場合、GLP-1製剤が費用的に難しい場合、あるいはGLP-1製剤に耐えられない場合です。どれに該当するかは医師がお伝えします。どれにも該当しない場合も含めて。
どの国も減量目的では承認していません
当院で扱う他の薬剤とは事情が異なるため、はっきり書いておきます。
リベルサスやマンジャロは、少なくともどこかの国で体重管理の承認があります（セマグルチドはウゴービとして、チルゼパチドはゼップバウンドとして）。メトホルミンは、主要な市場のどこにも体重管理の承認がありません。日本でも米国でもEUでも英国でも、糖尿病治療薬です。世界中のあらゆる減量目的の使用が適応外です。
省略できない安全性の話
メトホルミンの重篤なリスクは乳酸アシドーシスです。血中に乳酸が蓄積する状態で、まれではあるものの致死的となり得ます。米国の添付情報では枠組み警告が付されています。
腎機能の低下、肝疾患、心不全、多量の飲酒、重度の脱水、ヨード造影剤を用いる検査（一部のCT検査など）の前後で、リスクが大きく高まります。
血液検査を必須としている理由がこれです。腎機能は診察で評価できるものではありません。クレアチニンとeGFRを把握せずにメトホルミンを処方することは、適切な診療とは言えません。
また長期使用によりビタミンB12欠乏を生じることがあり、継続される場合は定期的な確認が必要になることがあります。
処方前に血液検査が必須です
当院では採血を行っておりません。お近くの内科で検査を受けていただき、結果の用紙をお持ちください。過去12か月以内の結果で結構です。項目は、肝機能（AST・ALT・γ-GTP）、腎機能（クレアチニン・eGFR）、膵酵素（アミラーゼ・リパーゼ）、甲状腺機能（TSH・FT4）、HbA1cと空腹時血糖です。

副作用
主なものは消化器症状です。下痢、悪心、腹部の不快感、金属味など。開始直後に強く、多くは改善していきます。食事とともに服用すると大きく軽減します。
直ちに受診してください：通常でない筋肉痛、呼吸のしにくさ、異常な眠気、強い腹痛、異常な寒気。いずれも乳酸アシドーシスの徴候である可能性があります。

メトホルミンは日本国内において2型糖尿病の治療薬として承認されています。減量目的での使用については承認されていません。減量目的での処方は適応外使用にあたります。プライベートクリニック恵比寿は100%自費診療のクリニックであり、保険診療は一切行っておりません。

国内の承認医薬品等の有無
あります。ウゴービ（セマグルチド）とゼップバウンド（チルゼパチド）が、日本において肥満症に対して承認されています。当院は両方を取り扱っています。どちらが適しているかは診察時に医師とご相談ください。
諸外国における安全性等に係る情報
メトホルミンは、米国・EU・英国においても体重管理の目的では承認されていません。いずれの主要市場においても糖尿病治療薬としての承認です。減量目的の使用は世界中どこでも適応外であり、減量に関するエビデンスはGLP-1受容体作動薬と比べて限定的です。
米国の添付情報では乳酸アシドーシスに関する枠組み警告が付されています。まれではあるものの致死的となり得る合併症で、腎機能低下・肝疾患・心不全のある方、多量に飲酒される方、ヨード造影剤を用いる検査を受ける方でリスクが高まります。その他の報告されている作用として、下痢、悪心、腹部不快感、金属味、および長期使用によるビタミンB12欠乏があり、定期的なモニタリングが必要となる場合があります。詳細は各国規制当局（米国FDA、EMA）の公式サイトをご確認ください。
以下に該当する方は使用できません
腎機能が著しく低下している方／重度の肝疾患のある方／治療を要する心不全のある方／多量に飲酒される方／妊娠中・授乳中の方、妊娠を計画中の方／ヨード造影剤を用いる検査の予定がある方。いずれも診察時に必ずお申し出ください。
