Rybelsus in Japan: The GLP-1 Tablet, and What It Is Not
Rybelsus is the option most people ask about when they do not want injections. It is a real medicine with real effects — but there is a widespread misunderstanding about it that we need to clear up before anything else.
The short version
- Rybelsus is oral semaglutide — the same active ingredient as Wegovy and Ozempic, in tablet form.
- In Japan it is approved for type 2 diabetes only. Weight-loss use is off-label.
- It is not the same as the overseas “Wegovy pill.” That product is 25 mg. Rybelsus in Japan is 3, 7, or 14 mg.
- Weight loss is generally more modest than with the injections.
- The dosing instructions are strict. Get them wrong and the medicine barely absorbs.
The misunderstanding worth clearing up first
In December 2025, the US FDA approved an oral semaglutide tablet for weight management, branded Wegovy. Coverage of that approval travelled widely, and people reasonably assumed it meant “the semaglutide pill works for weight loss.”
It does — at 25 mg.
Rybelsus in Japan is supplied at 3 mg, 7 mg, and 14 mg, for diabetes. The overseas weight-management regimen uses a substantially higher dose than anything available here under the Rybelsus name.
What this means for you. Taking Rybelsus off-label for weight loss is not equivalent to the regimen that was studied and approved overseas for that purpose. The published weight-loss figures from those trials do not transfer to it. Any clinic quoting “16% weight loss” alongside a Rybelsus prescription is mixing up two different products.
How it works
Semaglutide mimics GLP-1, a hormone released after eating that signals fullness to the brain and slows stomach emptying. The result is usually reduced appetite and earlier satiety.
Delivering a peptide by mouth is difficult — stomach acid destroys most of it. Rybelsus solves this with an absorption enhancer, which is why the dosing rules below are so specific.
How to take it — this part matters
Absorption is poor and inconsistent if the instructions are not followed exactly.
- Take it first thing after waking, on an empty stomach.
- Swallow with no more than half a glass of plain water (about 120 ml or less). Not tea, not coffee, not juice.
- Swallow the tablet whole. Do not split, crush, or chew it.
- Wait at least 30 minutes before eating, drinking anything else, or taking any other oral medicine or supplement.
Taking it with breakfast, or with coffee, or too close to other medicines, is the most common reason people conclude “it did nothing.” It usually did not get absorbed.
Who tends to choose it
Rybelsus is often the right starting point if you have a genuine aversion to needles, if you want to see how your body handles a GLP-1 before committing to injections, or if the monthly cost of injections is out of reach.
It is generally less effective for weight loss than the injectable options. If your goal is significant weight reduction and you meet Japan’s obesity-disease criteria, our physician may recommend Wegovy or Zepbound instead. We will tell you honestly which we think fits.
Blood tests are required before we prescribe
We do not draw blood at this clinic. Please have the tests done at a nearby internal medicine clinic (内科) and bring the printed results. Results from the past 12 months are acceptable: liver function (AST, ALT, γ-GTP), kidney function (creatinine, eGFR), pancreatic enzymes (amylase, lipase), thyroid function (TSH, FT4), and HbA1c with fasting glucose.

Side effects
Nausea, reduced appetite, diarrhoea, constipation, and abdominal discomfort are the common ones — most noticeable in the first weeks and after a dose increase, and usually settling.
Stop and seek medical attention immediately for severe, persistent abdominal pain radiating to the back (possible pancreatitis), or upper-right abdominal pain with fever (possible gallbladder problems).
Weight regain after stopping is expected, not a failure. Discuss a long-term plan with us before you start.
Required disclosures
This is an off-label, self-pay treatment
Rybelsus (active ingredient: semaglutide) is approved in Japan by the Ministry of Health, Labour and Welfare as a treatment for type 2 diabetes. It is not approved in Japan for weight management or the treatment of obesity (as of July 2026). Prescribing it for weight management is an off-label use. Private Clinic Ebisu is a 100% self-pay clinic and does not provide insurance-covered care of any kind.
How we obtain this medicine
We purchase Rybelsus through licensed Japanese pharmaceutical wholesalers, as genuine product manufactured and distributed in Japan by Novo Nordisk Pharma Ltd. We do not use personal imports or overseas-sourced product, and we will not prescribe or administer medicine you have imported yourself.
Are there approved alternatives in Japan?
Yes. Wegovy (semaglutide injection) and Zepbound (tirzepatide injection) are approved in Japan for obesity disease (肥満症). Wegovy contains the same active ingredient as Rybelsus. We stock both. Please discuss with our physician whether one of these would be more appropriate for you.
Safety information from other countries
Semaglutide is approved for weight management outside Japan — but not under the Rybelsus name, and not at Rybelsus doses. In the United States, semaglutide for weight management is marketed as Wegovy: as a once-weekly 2.4 mg injection, and — since December 2025 — as a once-daily 25 mg tablet. Rybelsus is supplied in Japan at 3 mg, 7 mg, and 14 mg for diabetes.
This difference matters. The oral semaglutide regimen approved overseas for weight management uses a substantially higher dose than Rybelsus. Taking Rybelsus off-label for weight loss is not equivalent to any regimen approved and studied for that purpose, and the published weight-loss results should not be assumed to apply to it.
US prescribing information for semaglutide carries a boxed warning regarding the risk of thyroid C-cell tumours. Reported adverse reactions include nausea, vomiting, diarrhoea, constipation, abdominal pain, reduced appetite, acute pancreatitis, gallbladder disease, and acute kidney injury following severe dehydration. Please consult the official websites of the relevant regulatory authorities (US FDA, EMA) for full prescribing information.
Do not use this medicine if you
are pregnant, breastfeeding or planning pregnancy; have a personal or family history of medullary thyroid carcinoma; have Multiple Endocrine Neoplasia syndrome type 2 (MEN2); have a history of pancreatitis; have severe gastrointestinal disease or gastroparesis; or have a history of an eating disorder.
Drug injury relief system
In the event of a serious adverse reaction, you will in principle not be eligible for Japan’s Relief System for Sufferers from Adverse Drug Reactions (医薬品副作用被害救済制度) or the Relief System for Infections Acquired through Biological Products (生物由来製品感染等被害救済制度). Harm arising from use outside an approved indication is not covered by these schemes.

リベルサス — 飲むGLP-1と、それが「違うもの」である理由
注射に抵抗のある方から最も多くご質問いただくのがリベルサスです。実際に効果のある医薬品ですが、その前にどうしても解いておきたい誤解が広く存在します。
要点
- リベルサスは経口セマグルチド。ウゴービ・オゼンピックと同じ有効成分の錠剤です。
- 日本では2型糖尿病に対してのみ承認されています。減量目的の使用は適応外です。
- 海外の「ウゴービ錠」とは別物です。あちらは25mg。日本のリベルサスは3mg・7mg・14mgです。
- 減量効果は、注射剤と比べると一般に穏やかです。
- 服用方法が厳密です。守らないとほとんど吸収されません。
まず解いておきたい誤解
2025年12月、米国FDAが体重管理を目的とした経口セマグルチド錠を承認しました。製品名はウゴービです。このニュースは広く伝わり、「セマグルチドの錠剤は減量に効く」と受け取られました。
効きます。25mgであれば。
日本のリベルサスは糖尿病用に3mg・7mg・14mgで供給されています。海外の体重管理用のレジメンは、リベルサスとして入手できるどの用量よりも大幅に高い用量を用いるものです。
この違いが意味すること。リベルサスを減量目的で適応外使用することは、海外でその目的のために研究され承認されたレジメンと同等ではありません。それらの試験で発表された減量幅の数値は、リベルサスには当てはまりません。リベルサスの説明に「16%の減量」といった数字を併記している医療機関は、2つの別製品を混同しています。
作用の仕組み
セマグルチドは、食後に分泌され満腹感を脳に伝え胃排出を遅らせるホルモンGLP-1を模倣します。結果として食欲が抑えられ、早い段階で満腹を感じるようになります。
ペプチドを経口で届けるのは困難で、大部分が胃酸で分解されてしまいます。リベルサスは吸収促進剤によってこれを解決しており、それゆえ下記の服用方法が非常に厳密になっています。
服用方法 — ここが重要です
指示どおりに服用しないと、吸収が不十分かつ不安定になります。
- 起床後すぐ、空腹の状態で服用します。
- コップ半分以下の水（約120ml以下）で飲みます。お茶、コーヒー、ジュースは不可です。
- 錠剤はそのまま飲み込みます。割る・砕く・噛むことはしないでください。
- 服用後30分以上は、食事・他の飲み物・他の内服薬やサプリメントを摂らないでください。
朝食と一緒に飲む、コーヒーで飲む、他の薬と近い時間に飲む。「効かなかった」と結論づけられるケースの大半がこれです。実際には吸収されていません。
どのような方に向くか
注射への抵抗が強い方、注射に踏み切る前にGLP-1が身体に合うか試したい方、注射剤の月額費用が難しい方には、リベルサスが適した出発点になることがあります。
ただし減量効果は注射剤より劣るのが一般的です。大きな減量を目標とされ、日本の肥満症の基準を満たす方には、医師がウゴービまたはゼップバウンドをおすすめする場合があります。どちらが適しているかは率直にお伝えします。
処方前に血液検査が必須です
当院では採血を行っておりません。お近くの内科で検査を受けていただき、結果の用紙をお持ちください。過去12か月以内の結果で結構です。項目は、肝機能（AST・ALT・γ-GTP）、腎機能（クレアチニン・eGFR）、膵酵素（アミラーゼ・リパーゼ）、甲状腺機能（TSH・FT4）、HbA1cと空腹時血糖です。

副作用
悪心、食欲低下、下痢、便秘、腹部の不快感が主なものです。開始直後と増量直後に強く出やすく、通常は落ち着きます。
直ちに使用を中止し受診してください：背部に放散する強く持続する腹痛（膵炎の可能性）、右上腹部痛と発熱（胆嚢の異常の可能性）。
中止後の体重再増加は想定される経過であり、失敗ではありません。開始前に長期的な方針をご相談ください。
必須の開示事項
適応外・自費診療です
リベルサス（有効成分：セマグルチド）は、日本国内において厚生労働省により2型糖尿病の治療薬として承認されています。体重管理や肥満症治療に対する使用については日本では承認されていません（2026年7月時点）。減量目的での処方は適応外使用にあたります。プライベートクリニック恵比寿は100%自費診療のクリニックであり、保険診療は一切行っておりません。
入手経路
リベルサスは国内の医薬品卸業者を通じて、ノボ ノルディスク ファーマ株式会社が国内で製造販売する正規品を仕入れています。個人輸入品・海外調達品は一切使用しておりません。患者様ご自身が輸入された医薬品の処方・投与もお受けできません。
国内の承認医薬品等の有無
あります。ウゴービ（セマグルチド注射剤）とゼップバウンド（チルゼパチド注射剤）が、日本において肥満症に対して承認されています。ウゴービの有効成分はリベルサスと同一です。当院は両方を取り扱っています。どちらが適しているかは診察時に医師とご相談ください。
諸外国における安全性等に係る情報
セマグルチドは日本国外で体重管理を目的として承認されていますが、リベルサスという名称でも、リベルサスの用量でもありません。米国において体重管理目的のセマグルチドはウゴービとして販売されており、週1回2.4mgの注射剤と、2025年12月以降は1日1回25mgの錠剤があります。日本のリベルサスは糖尿病用に3mg・7mg・14mgで供給されています。
この違いは重要です。海外で体重管理に承認された経口セマグルチドのレジメンは、リベルサスより大幅に高い用量を用います。リベルサスを減量目的で適応外使用することは、その目的で承認・研究されたいかなるレジメンとも同等ではなく、公表されている減量成績が当てはまると考えるべきではありません。
米国の添付情報では、甲状腺C細胞腫瘍のリスクに関する枠組み警告が付されています。報告されている副作用には、悪心、嘔吐、下痢、便秘、腹痛、食欲減退、急性膵炎、胆嚢疾患、重度脱水に伴う急性腎障害があります。詳細は各国規制当局（米国FDA、EMA）の公式サイトをご確認ください。
以下に該当する方は使用できません
妊娠中・授乳中の方、妊娠を計画中の方／甲状腺髄様癌の既往歴またはご家族に既往のある方／多発性内分泌腫瘍症2型（MEN2）の方／膵炎の既往歴のある方／重度の消化器疾患または胃不全麻痺のある方／摂食障害の既往歴のある方。

万が一重篤な副作用が出た場合には、原則として国の医薬品副作用被害救済制度および生物由来製品感染等被害救済制度の対象外となります。承認された効能効果の外での使用により生じた健康被害は、これらの制度の対象となりません。
